To Play a Sport

Congratulations on taking the 1st step to become a Student-Athlete at Seaside High School!
Seaside High's sports programs are traditionally known to field some of the best athletes in Monterey County. Our athletics program takes pride in developing Student-Athletes who are interested in taking their athletic and academic capabilities to the next level.
In order to be cleared for each sport, complete the steps below:
 
  1. Complete a Physical Exam Use this form: Pre-Participation Physical Evaluation

     

    This Athletic Pre-Participation Form must be completed by a parent and doctor prior to the start date of your sport. For your convenience, the Seaside Athletic Department will offer sports physicals for Winter season student-athletes on Wednesday, October 23rd at 2:45 pm in the Girls Varsity locker room. The cost is $25 each.

     

    IMPORTANT NOTE: The state of California only accepts physicals that are signed, stamped and dated from an MD, DO or PA (DC and RN signed physicals are not allowed).

  2. Receive and Complete a Sports Packet You will get this packet from the coach of the sport you are participating in. Upon completion of this packet, you will return the completed packet back to your coach. Sports packets will be handed out on Monday October 14th, you will pick up these packets from your coach. 
Attention: Parents of student athletes, each student that chooses to participate in a sport at Seaside High School is required to have medical coverage. If your student needs medical coverage, please go to https://www.myers-stevens.com/ to purchase coverage.
