Athletic Clearance

In order to be cleared for each sport, please complete the form below:

    This Athletic Pre-Participation Form must be completed by a parent and doctor prior to the start date of your sport. For your convenience, the Seaside Athletic Department will offer sports physicals at a date to be determined.

     

    IMPORTANT NOTE: The state of California only accepts physicals that are signed, stamped and dated from an MD, DO or PA (DC and RN signed physicals are not allowed).

Attention: Parents of student athletes, each student that chooses to participate in a sport at Seaside High School is required to have medical coverage. If your student needs medical coverage, please go to https://www.myers-stevens.com/ to purchase coverage.
 
2. Register online via SportsNet
 

Registration for all seasons is currently open. Parents and their student-athletes will need to visit the site, create an account and register for each sport you intend to play.

IMPORTANT NOTE: Please do not register for all sports — only for the sports you are planning to play. You can log back into your account to add a sport at a later date.

CIF Athletic Eligibility Forms must be completed and approved before a transfer student is considered eligible to compete in interscholastic athletics. These forms must be submitted to the Athletic Director and then approved by the CIF Central Coast Section office before a transfer student may participate in athletic contests. 
 
Transfer Paperwork
 
This information will enable the athletic department to complete and submit the transfer paperwork for approval to the CIF-CCS office for approval. A student is not eligible to compete in interscholastic contests until the forms are submitted and approved.
 
It is important that these forms are completed and submitted before the start of the school year or season, depending on the arrival of the transfer student. Delaying this process will also delay the ability for an athlete to compete. Your timeliness in this matter is much appreciated. 
Transfer Eligibility
You must contact the Athletic Director if:
  • You have attended another High School
  • You are NEW to Seaside HS
  • You do not live with your parents
  • You are a Foreign Exchange Student
